Background:

An anopenile urethral fistula (APUF) is a rare congenital anomaly.
This condition involves a connection between the rectum and the urethra.

Observation:

A newborn male presented with an imperforate anus and a perineal fistula.
Initial investigations revealed rectal gas below the ischium.
Colonourethrography confirmed a fistula between the rectum and the spongy urethra.

Findings:

The patient underwent colostomy at 1 day old and anterior sagittal anorectoplasty (ASARP) at 7 months.
The ASARP procedure involved dividing sphincter muscles, ligating the fistula, and relocating the rectum.
The fistula in the corpus spongiosum penis was intentionally left unresected.
Follow-up at 14 years showed no urinary or defecatory complications.

Implications:

ASARP offers an excellent surgical field for identifying fistulas and sphincter muscles.
Complete excision of the fistula from the corpus spongiosum may not be necessary for successful outcomes.
Understanding anorectal embryology, including urorectal septum development, is crucial for APUF etiology.
