In eukaryotic DNA replication, a single-stranded DNA fragment remains at the end of a chromosome after the removal of the final primer. This section of DNA cannot be replicated in the same manner as the rest of the strand because there is no 3’ end to which the newly synthesized DNA can attach. This non-replicated fragment results in gradual loss of the chromosomal DNA during each cell duplication. Background:

  • Bowen's disease, a form of squamous cell carcinoma in situ, can present with varied clinical and histological features.
  • Understanding the biological markers and metastatic potential of Bowen's disease is crucial for effective patient management.

Observation:

  • An 81-year-old Japanese woman presented with an abdominal erythematous plaque and nodule exhibiting bowenoid changes.
  • Biopsies confirmed bowenoid changes in the epidermis and epidermis to dermis.
  • Sentinel lymph node biopsy revealed atypical cell infiltration in two lymph nodes.

Findings:

  • High telomerase activity was detected in the erythematous plaque using a telomeric repeat amplification protocol assay.
  • The lesion was surgically excised with a 3-cm margin, followed by complete regional lymph node dissection.
  • Sentinel lymph node biopsy (SNB) successfully identified early lymph node metastases.

Implications:

  • High telomerase activity may be associated with aggressive forms of Bowen's disease.
  • This case suggests that some instances of Bowen's disease can progress to invasive Bowen's carcinoma.
  • SNB is a valuable tool for staging and detecting early lymphatic spread in cases of Bowen's disease with concerning features.
