Summary

Liver transplantation (LT) can be a life-saving option for pregnant women with severe HELLP syndrome. This study reports on eight patients, showing significant survival rates after LT for this rare but serious pregnancy complication.

Main Results:

  • All eight patients received cadaveric grafts; mean interval from delivery to LT was 7 days.
  • High pre-transplant severity scores (mean CTP 13.1, MELD 40) indicated critical illness.
  • Post-LT survival rates at 1, 5, and 10 years were 88%, 88%, and 65% for patients, and 64%, 64%, and 48% for grafts, respectively.
  • Complications included biliary leaks and reoperations; one early and one late death occurred.

Conclusions:

  • Liver transplantation is a viable, albeit complex, treatment for liver failure due to HELLP syndrome.
  • Early recognition and timely transfer to a transplant center are crucial for optimal outcomes in these critically ill patients.
