    Spinal Hernia Repair and Cauda Equina Repositioning After Lumbar Decompression under Three-Dimensional Microscopy: A Case Report and Literature Review

    Background:

    • A 62-year-old male with a history of low-grade non-Hodgkin's lymphoma presented with low back and thigh pain.
    • Initial symptoms were attributed to chronic low back pain and treated with manual therapy.

    Observation:

    • The patient developed a left-sided limp and facial droop (Bell's palsy) despite initial improvement.
    • Lumbar spine CT showed disc bulging at L4/5, initially suggesting a disc lesion.

    Findings:

    • MRI revealed leptomeningeal disease affecting cranial nerves, cauda equina, and vertebral bodies (L1, L5).
    • This widespread leptomeningeal carcinomatosis was the direct cause of the patient's death three weeks post-diagnosis.

    Implications:

    • Leptomeningeal disease should be considered in cancer patients presenting with back pain and neurological symptoms.
    • This case highlights the importance of advanced imaging in diagnosing rare but aggressive conditions mimicking common ailments.
