Background:

  • Appendiceal viral infections are rarely documented histologically.
  • Adenovirus is linked to mesenteric adenitis and intussusception, but not typically acute appendicitis without these conditions.

Observation:

  • A retrospective review of 877 appendectomies (2001-2005) identified 94 pathologically negative cases with clinical acute appendicitis.
  • Sixty-three of these negative cases showed lymphoid hyperplasia and were tested for adenovirus.

Findings:

  • Two cases (a 6-year-old male and a 5-year-old female) tested positive for adenovirus.
  • These positive cases exhibited epithelial proliferation and viral inclusions, without intussusception.

Implications:

  • Adenovirus infection can clinically present as acute appendicitis, even without intussusception.
  • Histological evaluation of negative appendectomies for viral changes, including adenovirus, is recommended.
  • Adenovirus may contribute to the development of acute appendicitis.
