  • Uterine perforation is a rare complication of IUD insertion.

Observation:

  • An intraperitoneal IUD was incidentally discovered during lumbar spine X-rays.
  • Computed tomography (CT) confirmed the device's abdominal location.

Findings:

  • The intraperitoneal IUD was secondary to an asymptomatic uterine perforation.
  • The perforation occurred at the time of IUD insertion 17 years prior.

Implications:

  • This case highlights the importance of imaging in identifying retained foreign bodies.
  • Delayed diagnosis of uterine perforation can occur.
  • Long-term asymptomatic complications of IUDs are possible.
