Evidence for prolonged antibiotic use to prevent childhood urinary tract infections (UTIs) and kidney scarring is weak. More research is needed, especially for children with vesicoureteral reflux (VUR).
Area of Science:
- Pediatric Nephrology
- Infectious Diseases
- Evidence-Based Medicine
Background:
- Urinary tract infections (UTIs) are common in children.
- Antibiotic prophylaxis is frequently recommended for preventing recurrent UTIs and renal scarring, particularly in children with vesicoureteral reflux (VUR).
Purpose of the Study:
- To review the scientific evidence supporting the use of antibiotic prophylaxis for UTIs in children.
- To evaluate the quality of evidence regarding antibiotic prophylaxis efficacy in preventing UTIs and renal scarring.
- To identify gaps in the current research, especially concerning children with VUR.
Main Methods:
- Systematic review of existing scientific literature.
- Analysis of studies on antibiotic prophylaxis in pediatric UTIs.
- Assessment of evidence quality and relevance to different pediatric populations.
Main Results:
- The available scientific evidence supporting prolonged antibiotic use for UTI prevention and renal scar development in children is scarce and of low quality.
- Most studies have focused on children with anatomically normal urinary tracts.
- There is a significant lack of information on the effectiveness of antibiotic prophylaxis in children with VUR, a group for whom it is commonly recommended.
Conclusions:
- The current evidence base for antibiotic prophylaxis in pediatric UTIs is insufficient to justify its widespread use.
- Clinical practice guidelines recommending antibiotic prophylaxis, especially for children with VUR, should be re-evaluated in light of the limited evidence.
- Further high-quality research is necessary to determine the true efficacy and appropriate use of antibiotic prophylaxis in pediatric UTIs.
