Purpose of the Study:

  • To investigate the association between a history of depression and/or PTSD and all-cause mortality.
  • To analyze mortality risk in primary care patients over an average 2-year follow-up period.

Main Methods:

  • A cohort of 35,715 primary care patients from Department of Veterans Affairs medical centers was studied.
  • Depression and PTSD status were determined via self-report and electronic health records.
  • Mortality data were obtained from official Veterans Affairs databases.

Main Results:

  • Patients with a history of depression only (n=6876) showed an increased risk of death (HR=1.17; 95% CI=1.06-1.28) after adjustments.
  • Patients with PTSD only (n=748) did not have a significantly increased risk of death (HR=0.84; 95% CI=0.63-1.13).
  • Patients with both depression and PTSD (n=3762) had a borderline increased risk of death after initial adjustments, which was not significant after further adjustments.

Conclusions:

  • A prior diagnosis of depression, independent of PTSD, is associated with an increased risk of all-cause mortality in primary care patients.
  • Posttraumatic stress disorder (PTSD) alone was not significantly associated with increased mortality in this cohort.
  • These findings underscore the importance of managing depression in veteran populations to mitigate mortality risks.
