Summary

Off-pump left mini-thoracotomy (LME) coronary artery bypass grafting using the left internal thoracic artery (LITA) to the left anterior descending artery (LAD) demonstrates safety and efficacy. This minimally invasive approach offers reduced hospital stays and low complication rates for eligible patients.

Area of Science:

  • Cardiovascular Surgery
  • Minimally Invasive Cardiac Surgery
  • Interventional Cardiology

Background:

  • Stent restenosis is a frequent complication following angioplasty.
  • Left internal thoracic artery (LITA) to left anterior descending artery (LAD) anastomosis is associated with superior outcomes.
  • Minimally invasive techniques are increasingly explored for coronary artery bypass grafting.

Purpose of the Study:

  • To present patient selection, operative technique, and outcomes of off-pump left mini-thoracotomy (LME) coronary surgery.
  • To evaluate the feasibility and safety of LME for LITA-LAD bypass grafting as a pilot study.

Main Methods:

  • Eighteen patients underwent off-pump LME for LITA-LAD or sequential anastomoses.
  • Grafts were skeletonized, and anastomoses were performed using 7-0 polypropylene sutures.

  • A specialized access device facilitated coronary artery approach and stabilization.
  • Main Results:

    • No deaths, conversions to sternotomy, transfusions, or elevated enzyme levels were reported.
    • Patients experienced short hospital stays (3-5 days) and rapid return to daily activities.
    • Angiography in six patients confirmed patent grafts; one graft occlusion required stenting.

    Conclusions:

    • Off-pump LME coronary surgery is associated with low morbid-mortality, short hospital stays, and no transfusions.
    • The procedure was technically feasible and facilitated by appropriate instruments and surgeon's dexterity.
    • Further randomized trials are warranted to establish long-term clinical and economic benefits for LME as a primary surgical option.
