Purpose of the Study:

  • To investigate the association between erectile dysfunction (ED) and various psychopathological symptoms.
  • To quantify the contribution of organic, relational, and intrapsychic domains to ED using the SIEDY Structured Interview.
  • To screen for mental disorders in non-psychiatric settings using the Middlesex Hospital Questionnaire.

Main Methods:

  • A cohort of 1,388 male patients with ED (mean age 51 years) was studied.
  • Participants completed the SIEDY Structured Interview and the Middlesex Hospital Questionnaire.
  • Hormonal, biochemical, and sexual function parameters were assessed.

Main Results:

  • Psychiatric symptoms showed distinct associations with ED domains: depressive and phobic-anxiety symptoms linked to the relational domain; somatization to the organic domain; free-floating anxiety, obsessive-compulsive, and phobic symptoms to the intrapsychic domain.
  • Depressive symptoms correlated with hypogonadism, low intercourse frequency, hypoactive sexual desire (HSD), and relationship conflicts.
  • Somatization symptoms were associated with poorer erectile function, while phobic and obsessive-compulsive symptoms related to better relational functioning and lower HSD prevalence.

Conclusions:

  • Findings challenge the belief that psychiatric symptoms are uncommon in ED patients.
  • Systematic screening for psychopathological distress in ED patients is recommended.
  • Early detection and management of associated psychiatric issues can mitigate sexual disturbances.
