Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy01:26

Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Summary

Emergency left colon and rectal surgery remains debated. Single-stage total or subtotal colectomy in 32 patients showed good outcomes, with complications including anastomotic leakage and infections, but offered oncological radicality.

Area of Science:

  • Colorectal surgery
  • Surgical oncology
  • Emergency medicine

Context:

  • The optimal surgical approach for left-sided colonic and rectal emergencies is not standardized.
  • Emergency intestinal occlusion is a common surgical presentation.
  • This study evaluates a specific surgical approach for left-sided colonic emergencies.

Purpose:

  • To assess the outcomes of single-stage radical procedures (total or subtotal colectomy) for left-sided colonic and rectal emergencies.
  • To compare the incidence of complications and functional results with existing literature.
  • To determine the oncological safety and feasibility of radical resection in emergency settings.

Summary:

  • A retrospective analysis of 32 patients undergoing total or subtotal colectomy for left-sided colonic emergencies between 1998-2007.
  • The primary indication for surgery was intestinal occlusion, with left colic carcinoma being the most frequent postoperative diagnosis.
  • The radical procedure was performed within 24 hours of admission, even in cases with metastatic disease.

Impact:

  • Total or subtotal colectomy provides oncological radicality and satisfactory functional outcomes for left-sided colonic emergencies.
  • The study reported a 65.62% good evolution rate, with complications including anastomotic leakage (6.25%) and parietal infections (21.87%).
  • Postoperative diarrhea was noted as a disadvantage, though it gradually decreased over time.
