Ophthalmic Drug Delivery Systems01:23

Ophthalmic Drug Delivery Systems

Ophthalmic drug delivery faces major limitations due to poor absorption across the corneal membrane. This process is primarily driven by diffusion and is influenced by two main factors: the physicochemical properties of the drug and tear drainage. Most ophthalmic drugs, such as pilocarpine, epinephrine, atropine, and local anesthetics, are weak bases. They are typically formulated at an acidic pH to enhance chemical stability. However, this leads to high ionization, reducing their ability to...

Summary

Injectable calcium hydroxylapatite offers a safe and effective method for correcting anophthalmic enophthalmos by augmenting orbital volume. This semipermanent filler provides lasting aesthetic improvement for over a year with minimal loss.

Area of Science:

  • Ophthalmology
  • Plastic Surgery
  • Biomaterials

Background:

  • Anophthalmic enophthalmos presents aesthetic and functional challenges.
  • Existing orbital volume augmentation methods may be invasive or costly.

Purpose of the Study:

  • To introduce and evaluate injectable calcium hydroxylapatite for orbital volume augmentation.
  • To assess the safety, efficacy, and longevity of this new technique for anophthalmic enophthalmos correction.

Main Methods:

  • Retrospective review of 4 consecutive patients.
  • Injectable calcium hydroxylapatite (Radiesse) was placed in the extraconal space.
  • Measurements of enophthalmos using Hertel exophthalmometry were taken pre- and post-procedure.

Main Results:

  • A mean reduction of 2.75 mm in enophthalmos was observed.
  • All patients showed clinical and aesthetic improvement lasting over one year.
  • One case of peribulbar hemorrhage occurred, resolving without sequelae.

Conclusions:

  • Injectable calcium hydroxylapatite is a safe, simple, and cost-effective option for orbital volume deficiency.
  • The semipermanent filler provides lasting augmentation (1+ year) with minimal volume loss.
  • The procedure is repeatable, titratable, and can be performed under local anesthesia in an office setting.
