Background:

Spontaneous biloma is a rare clinical occurrence.
The biliary system is a complex network responsible for bile transport.

Observation:

A case of subcapsular biloma was identified in an elderly patient.
The patient had a nonobstructed biliary channel.
No history of surgery, instrumentation, or trauma was noted.

Findings:

Computed tomography (CT) and magnetic resonance imaging (MRI) were utilized for diagnosis.
The biloma was located beneath the liver capsule.

Implications:

This represents a potential case of idiopathic spontaneous subcapsular biloma.
Further research may elucidate the underlying mechanisms of spontaneous biloma formation.
