Abstract

Insights

High doses of sildenafil combined with nitroglycerin (NTG) significantly increase the defibrillation threshold (DFT). This effect on DFT is primarily due to sildenafil, not NTG.

Area of Science:

  • Cardiology
  • Pharmacology
  • Critical Care Medicine

Background:

  • Sildenafil citrate at supra-therapeutic concentrations can reduce defibrillation efficacy.
  • The impact of combining therapeutic sildenafil levels with nitroglycerin (NTG) on defibrillation is unknown.

Purpose of the Study:

  • To determine if therapeutic sildenafil levels, when combined with NTG, elevate the defibrillation threshold (DFT).

Main Methods:

  • Twenty-four pigs were randomized into four groups.
  • Sildenafil (50mg or 100mg) or saline was infused, followed by NTG administration.
  • Defibrillation threshold (DFT) was measured before and after NTG infusion.

Main Results:

  • Therapeutic sildenafil with NTG did not alter DFT.
  • Supra-therapeutic sildenafil with NTG significantly increased DFT (p < 0.004).
  • Saline did not affect DFT, alone or with NTG.

Conclusions:

  • A supra-therapeutic dose of sildenafil in combination with NTG significantly elevates the DFT.
  • The increase in DFT appears to be driven by sildenafil, not NTG.

Treatment for Pulmonary Arterial Hypertension: Phosphodiesterase Inhibitors

Phosphodiesterase 5 (PDE5) inhibitors are potent enzymes that function to hydrolyze cyclic nucleotides to their corresponding 5' monophosphates. Their unique biochemical properties have been applied in treating Pulmonary Arterial Hypertension (PAH).
Among the PDE5 inhibitors, sildenafil (Revatio) stands out as a competitive and selective inhibitor. It operates by elevating cellular levels of cGMP and augmenting signaling through the cGMP-PKG pathway, promoting vasodilation. Upon oral...

Antianginal Drugs: Nitrates and β-Blockers

In cardiovascular health, antianginal drugs combat angina pectoris — a condition marked by chest pain owing to diminished blood flow to the heart.
Organic nitrates,  such as nitroglycerin, play a pivotal role. Once metabolized, they liberate nitric oxide, a molecular marvel. Nitric oxide triggers guanylyl cyclase and augments cGMP production. This biochemical cascade orchestrates the relaxation of vascular smooth muscles, ushering in vasodilation and enhancing coronary blood flow. Administered...

Cardiopulmonary Resuscitation III: AED Use

Introduction to AEDAn Automated External Defibrillator (AED) is a portable medical device that analyzes the heart's rhythm and, if necessary, delivers an electrical shock to help the heart re-establish an effective rhythm during sudden cardiac arrest (SCA). SCA occurs when the heart suddenly and unexpectedly stops beating, leading to a loss of blood flow to the brain and other vital organs. In such emergencies, time is of the essence, and using an AED, combined with Cardiopulmonary...

Cardiopulmonary Resuscitation IV: Pharmacological Management

Pharmacologic intervention is crucial in treating cardiac arrest patients during ACLS or Advanced Cardiovascular Life Support. The ACLS algorithms guide the administration of specific drugs based on the patient's cardiac arrest rhythm, which includes pulseless ventricular tachycardia (VT), ventricular fibrillation (VF), asystole, and pulseless electrical activity (PEA).EpinephrineIndication: Epinephrine is the first-line drug for all cardiac arrest rhythms.Mechanism of Action: Epinephrine...

Heart Failure Drugs: Inotropic Agents

Positive inotropic agents are commonly used as the first line of treatment for heart failure. One such agent is digoxin, derived from the genus Digitalis, which has been known for centuries but effectively utilized since 1785. However, these cardiac glycosides can have potentially toxic effects due to their mechanism of action, which involves inhibiting Na+/K+-ATPase and increasing contractility. Digoxin is absorbed orally and distributed in various tissues, including the CNS. It has a long...
