This study reports a rare case of amylase-producing multiple myeloma in an 80-year-old man. The condition was confirmed through immunocytochemistry, cell culture, and gene expression analysis.

Background:

Observation:

  • An 80-year-old male presented with acute thrombocytopenia and renal failure, diagnosed as Bence Jones (lambda)-type multiple myeloma with sepsis.
  • Elevated serum amylase (S-amylase) activity was noted on admission.
  • Myeloma cells in pleural effusion demonstrated cytoplasmic amylase positivity and secreted S-amylase in vitro.

Findings:

  • The patient's myeloma cells expressed S-amylase, confirmed by immunocytochemistry, cell culture supernatant analysis, and RT-PCR.
