Surgical Angiogenesis in Porcine Tibial Allotransplantation: A New Large Animal Bone Vascularized Composite Allotransplantation Model

Background:

  • Bone marrow aspirate (BMA) combined with particulate allografts and xenografts promotes new bone growth.
  • Allograft bone blocks offer clinical advantages in specific situations.
  • This study explores cell-based therapy using BMA-derived stem cells within allograft corticocancellous bone blocks for bone regeneration.

Observation:

  • A technique was developed to combine BMA with block allografts for osseous defect repair.
  • Five patients with atrophic maxillary and mandibular ridges received grafts using allograft bone blocks saturated with BMA.
  • Histologic and histomorphometric analysis of a bone core taken 4 months post-implantation was performed.

Findings:

  • Grafts integrated into recipient bone within 4 to 8 months in all cases.
  • Implants placed at all 5 grafted sites achieved successful osseointegration.
  • Histomorphometric analysis revealed the graft comprised 54% bone and 46% marrow, with 89% of the bone being vital.

Implications:

  • Impregnating allograft bone blocks with BMA stimulates endogenous bone formation.
  • BMA aspiration is a minimally invasive technique compared to autogenous bone harvesting.
  • This approach provides predictable results and is cost-effective for bone regeneration.
