The Doloplus-2 tool shows poor criterion validity for assessing pain in cognitively impaired patients. Specific administration skills are needed for this behavioral pain assessment tool.

Background:

  • The Doloplus-2 is a tool for behavioral pain assessment in patients with cognitive impairment.
  • Limited data exists on the psychometric properties of the Doloplus-2.

Purpose of the Study:

  • To test the criterion validity of the Doloplus-2.
  • To assess the inter-rater reliability of the Doloplus-2.
  • To explore study designs for validating behavioral pain assessment tools.

Main Methods:

  • Fifty-one nursing home patients and 22 geriatric hospital patients with cognitive impairment were included.
  • An expert rated pain using a numerical rating scale as the criterion.
  • The Doloplus-2 was administered by nurses; inter-rater reliability was assessed in 16 patients.

Main Results:

  • No significant association was found between Doloplus-2 scores and expert pain ratings (R2 = 0.02).
  • A subgroup of 16 patients assessed by an expert nurse showed an association (R2 = 0.54).
  • Inter-rater reliability for Doloplus-2 was 0.77; expert ratings showed satisfactory agreement (ICC 0.74).

Conclusions:

  • The study faced challenges due to cognitive impairment in patients.
  • Results do not support the Doloplus-2's validity in its current form.
  • The Doloplus-2 requires specific administration skills for reliable use.
