Summary

Laparoscopic antireflux surgery is safe and effective for elderly patients (70+) with gastroesophageal reflux or large paraesophageal hernias. Age should not be a barrier to this beneficial surgical intervention.

Background:

  • Gastroesophageal reflux and paraesophageal hernias are prevalent in the elderly.
  • Older patients are often excluded from surgical interventions due to age concerns.

Purpose of the Study:

  • To evaluate the outcomes of laparoscopic antireflux surgery in patients aged 70 years and older.
  • To assess the safety and efficacy of this procedure in an elderly population.

Main Methods:

  • A prospectively maintained clinical database was utilized.
  • Patients aged 70 years or older undergoing laparoscopic antireflux surgery were identified.
  • Outcomes were determined through follow-up assessments.

Main Results:

  • Two hundred ten elderly patients were included (129 with large paraesophageal hernias, 81 with reflux).
  • Surgery for hernias was longer (109 vs. 72 min) and had higher conversion rates (11.6% vs. 4.4%) than for reflux alone.
  • Significant improvement in heartburn and dysphagia symptoms, with high patient satisfaction.

Conclusions:

  • Laparoscopic antireflux surgery demonstrates satisfactory clinical outcomes in patients aged 70 and older.
  • Age alone should not preclude elderly patients from undergoing this surgical procedure.
