Area of Science:

  • Neurosurgery
  • Radiation Oncology
  • Spinal Surgery

Background:

  • Vertebral hemangiomas are benign vascular tumors of the spine.
  • Symptomatic presentation, particularly with neurologic deficit, is uncommon.
  • Various treatment options exist, but optimal management remains debated.

Purpose of the Study:

  • To report personal experience with surgical and radiation-based treatments for symptomatic vertebral hemangiomas.
  • To propose a treatment algorithm for managing these rare spinal tumors.
  • To review existing literature and theoretical considerations for treatment efficacy.

Main Methods:

  • Retrospective review of five patients with symptomatic vertebral hemangiomas.
  • Surgical intervention (laminectomy) followed by radiation therapy in four patients.
  • Radiation therapy alone in one patient.

Main Results:

  • Personal experience with laminectomy and radiation therapy in four patients.
  • Radiation therapy alone in one patient.
  • Evaluation of outcomes to inform treatment recommendations.

Conclusions:

  • Laminectomy followed by radiation therapy is a viable option for symptomatic vertebral hemangiomas with neurologic compromise.
  • A proposed treatment strategy integrates surgical decompression and adjuvant radiation.
  • Further research is needed to establish definitive treatment guidelines for this rare condition.
