Background:

  • Skeletal class III malocclusion presents unique challenges in orthodontic and surgical management.
  • Temporomandibular joint (TMJ) disorders can be influenced by skeletal discrepancies and surgical interventions.

Purpose of the Study:

  • To assess the impact of orthognathic surgery on the articular disc position within the temporomandibular joint.
  • To evaluate changes in temporomandibular disorder symptoms in skeletal class III patients following orthognathic surgery.

Main Methods:

  • Clinical examination and magnetic resonance imaging (MRI) were used for pre- and postoperative evaluation.
  • Thirty-six patients with skeletal class III malocclusion underwent evaluation after undergoing mandibular setback surgery (sagittal split ramus osteotomy).

Main Results:

  • Mandibular setback surgery did not result in a statistically significant change in articular disc position.
  • A trend towards posterior displacement of the articular disc was observed post-surgery.

Conclusions:

  • Orthognathic surgery, specifically mandibular setback, appears to have a limited significant effect on the articular disc's position in skeletal class III patients.
  • Further research may explore long-term TMJ implications and symptom correlation.
