Background:

Observation:

  • A 30-year-old male presented with lymphadenopathy, negative inflammatory markers, and unremarkable initial infectious workup.
  • Bartonella henselae was not detected via blood or lymph node cultures, nor PCR, potentially due to prior antibiotic treatment.
  • Diagnosis was established based on cat contact history, clinical presentation, and necrotizing granulomatous inflammation on lymph node biopsy.

Findings:

  • The patient experienced complete resolution of lymphadenopathy after a five-day course of oral azithromycin.
  • Despite negative microbiological and PCR results, clinical and histopathological findings confirmed CSD.

Implications:

  • CSD should be considered in adult patients with unexplained lymphadenopathy and a history of cat exposure.
  • This case underscores the importance of clinical suspicion and patient history in diagnosing CSD, especially in adults where it is less common.
