Summary

Gamma Knife thalamotomy (GKT) offers a safe and effective treatment for essential tremor when other therapies fail. This radiosurgery significantly reduces tremor and improves handwriting, especially for elderly patients unsuitable for other surgical options.

Background:

  • Essential tremor is a common neurological disorder causing disabling tremors.
  • Medically refractory essential tremor often requires surgical intervention.
  • Gamma Knife thalamotomy (GKT) presents an alternative for patients unsuitable for traditional surgery.

Purpose of the Study:

  • To evaluate the efficacy and safety of Gamma Knife thalamotomy (GKT) for essential tremor.
  • To assess outcomes in patients with medically refractory essential tremor who were not candidates for open surgery.

Main Methods:

  • Thirty-one patients with disabling essential tremor underwent GKT targeting the nucleus ventralis intermedius.
  • A single 4-mm isocenter delivered 130 or 140 Gy.
  • Tremor and handwriting were assessed using the Fahn-Tolosa-Marin scale pre- and post-radiosurgery.

Main Results:

  • Significant reduction in tremor scores (mean 3.7 to 1.7, p < 0.000015) and handwriting scores (mean 2.8 to 1.7, p < 0.0002) observed.
  • 69% of patients improved in both tremor and writing, 23% improved in tremor only.
  • Low rates of adverse events, including mild, delayed hemiparesis and speech impairment in one patient.

Conclusions:

  • Gamma Knife thalamotomy is a safe and effective treatment for medically refractory essential tremor.
  • GKT is particularly beneficial for elderly patients or those with comorbidities ineligible for other surgical procedures.
  • Patients should be informed about the potential for delayed neurological deficits, although rare.
