Background:

  • Herpes zoster infection, particularly in the lumbo-sacral region, can lead to voiding dysfunction.
  • This complication is rare but significant, especially in immunocompromised individuals.

Observation:

  • A retrospective analysis of 13 herpes zoster patients with voiding dysfunction was conducted.
  • Twelve patients had underlying conditions, including hematopoietic malignancies and hematopoietic stem cell transplant (HSCT) recipients.
  • Urinary retention onset and duration correlated with rash severity and underlying malignancy.

Findings:

  • Patients with severe herpes zoster rash or hematopoietic malignancy experienced delayed urinary retention onset.
  • Severe rash was also associated with longer durations of urinary retention.
  • Acute urinary retention was observed as late as 19 days post-rash onset in one case.

Implications:

  • Healthcare providers should monitor immunocompromised patients with lumbo-sacral herpes zoster for delayed urinary retention.
  • Early recognition and management are crucial, even after the resolution of cutaneous symptoms.
  • This highlights the importance of considering neurological complications of herpes zoster in urological dysfunction.
