Secondary syphilis can present unusually with skin lesions in body folds. Early consideration of syphilis is vital for patients with risk factors and these specific skin manifestations.

Area of Science:

  • Dermatology
  • Infectious Diseases
  • Syphilology

Background:

  • Secondary syphilis is a systemic manifestation of Treponema pallidum infection.
  • Typical secondary syphilis lesions are widespread, but atypical presentations occur.

Observation:

  • A case of secondary syphilis presented with unusual papules in intertriginous areas (skin folds).
  • Initial skin biopsy was nondiagnostic, delaying definitive diagnosis.

Findings:

  • Serologic tests ultimately confirmed the diagnosis of secondary syphilis.
  • The patient had an at-risk sexual history.

Implications:

  • Clinicians should consider syphilis in the differential diagnosis of patients with atypical skin lesions, particularly in intertriginous zones.

Related Experiment Videos

  • A high index of suspicion is crucial for patients with risk factors for sexually transmitted infections.
  • Prompt serologic testing is essential when syphilis is suspected, even with nondiagnostic initial biopsies.
