Infective endocarditis (IE) caused by Streptococcus bovis (S. bovis) is an emerging disease, showing a significant increase and distinct characteristics compared to viridans group streptococci (SVG) IE.
Area of Science:
- Infectious Diseases
- Cardiology
- Microbiology
Background:
- Infective endocarditis (IE) is a serious infection affecting heart valves.
- Streptococcus bovis (S. bovis) and viridans group streptococci (SVG) are common causes of IE.
- Understanding the distinct features of IE caused by different pathogens is crucial for diagnosis and treatment.
Purpose of the Study:
- To evaluate the characteristics of IE caused by S. bovis.
- To compare S. bovis IE with IE caused by SVG.
- To identify trends and associations of S. bovis IE over time.
Main Methods:
- Prospective study over 18 years (1988-2005).
- Inclusion of 55 consecutive cases of S. bovis IE and 41 cases of SVG IE.
- Comparison of clinical, microbiological, and epidemiological data between the two groups.
Main Results:
- S. bovis caused 24% of IE cases, with a 358% increase in the second study period.
- S. bovis IE was associated with older males, less predisposing cardiopathy, and more bivalvular involvement and spondylitis.
- A high association (53%) with colonic tumors and increased antibiotic resistance (erythromycin, clindamycin, cotrimoxazole) were observed for S. bovis IE.
Conclusions:
- S. bovis IE is an emergent disease with unique clinical and epidemiological features.
- The findings highlight the importance of investigating for colonic tumors in patients with S. bovis IE.
- Increased antibiotic resistance in S. bovis necessitates careful treatment selection.
