Progressive brown corneal pigmentation in a patient with conjunctival melanosis was linked to iris melanoma. Pigment granules from iris tumors entered the cornea via the endothelium, accumulating in melanophages.

Area of Science:

  • Ophthalmology
  • Pathology

Background:

  • A rare case of progressive brown corneal pigmentation is presented.
  • The patient had a history of primary acquired melanosis of the conjunctiva.

Observation:

  • A 36-year-old female developed corneal stromal pigmentation adjacent to a conjunctival lesion.
  • Over 8 years, corneal pigmentation progressed, impacting visual acuity.
  • Iris nodules, trabecular meshwork pigmentation, and endothelial precipitates were observed.

Findings:

  • Histopathology confirmed a compound nevus with atypia and acquired melanosis of the conjunctiva.
  • Post-keratoplasty, corneal pigmentation increased, and iris melanomas developed.
  • Melanophages accumulated on the corneal graft endothelium, with pigmented cells in the host cornea stroma.

Related Experiment Videos

Implications:

  • Iris tumors were identified as the source of corneal pigment granules.
  • Pigment transport occurred through the corneal endothelium into stromal melanophages.
  • The process mimicked corneal hemochromatosis clearance via the limbus.
