Area of Science:

  • Minimally Invasive Surgery
  • Robotic Surgery
  • Pediatric Urology

Background:

  • Robot-assisted laparoscopic surgery offers advantages over conventional laparoscopy in pediatric urology.
  • Challenges arise with the da Vinci Surgical System in confined surgical spaces.

Purpose of the Study:

  • To objectively evaluate the impact of workspace size on robotic-assisted laparoscopic surgical skills.
  • Assessing performance with the da Vinci Surgical System in varying cavity dimensions.

Main Methods:

  • Seven assessors performed standardized laparoscopic drills in cubic boxes ranging from 40 to 150 mm edge size.
  • Drills were based on the McGill Inanimate System for Training and Evaluation of Laparoscopic Skills.
  • Performance was measured by drill achievement, time to completion, and a global score.

Main Results:

  • Significant collisions occurred in workspaces with 40-45 mm edges, hindering drill completion.
  • Drills were challenging but possible in 50-60 mm workspaces without collisions.
  • Easier and uniform drill accomplishment was observed in workspaces 70 mm and larger.

Conclusions:

  • Surgeon performance with the da Vinci Surgical System is restricted in small workspaces.
  • Workspace size critically impacts surgeon performance in robotic-assisted procedures.
  • Statistical analysis confirmed the significant effect of workspace dimensions on surgical outcomes.
