Background:

  • Polymyalgia rheumatica (PMR) is an inflammatory condition often causing neck stiffness and pain.
  • The exact cause of neck discomfort in PMR is not fully understood.
  • Cervical interspinous bursae have not been extensively studied in PMR.

Purpose of the Study:

  • To investigate the presence and severity of cervical interspinous bursitis in new-onset, untreated polymyalgia rheumatica patients using Magnetic Resonance Imaging (MRI).
  • To compare the prevalence of cervical bursitis in PMR patients with that in control groups.

Main Methods:

  • MRI of the cervical spine was performed on 12 new PMR patients and 13 control patients (fibromyalgia, cervical osteoarthritis, spondyloarthritis).
  • Cervical interspinous bursitis was assessed using a semiquantitative scale (0-3).
  • Lumbar spine MRI was conducted in two PMR patients with pelvic girdle pain.

Main Results:

  • All 12 PMR patients showed MRI evidence of cervical interspinous bursitis.
  • Moderate to marked cervical bursitis (grade ≥2) was significantly more frequent in PMR patients (83.3%) than in controls (30.7%, p=0.015).
  • Cervical bursitis occurred at C5-C7 interspaces in all affected individuals; lumbar bursitis was noted in two PMR patients.

Conclusions:

  • Cervical interspinous bursitis is a likely cause of neck discomfort in polymyalgia rheumatica.
  • The significant inflammatory involvement of cervical bursae in PMR supports the hypothesis that PMR affects extra-articular synovial structures.
