  • Four male patients experienced BPPV symptoms hours after intensive mountain biking without head trauma.
  • Positional maneuvers indicated involvement of the posterior and horizontal semicircular canals (SCCs).

Findings:

  • BPPV was attributed to otoconia displacement in the SCCs, likely caused by repetitive head accelerations during biking.
  • Symptoms resolved spontaneously or with vestibular rehabilitation in all patients.

Implications:

  • Mountain biking, particularly off-road, is identified as a potential cause of BPPV.
  • This suggests a new category of non-traumatic BPPV related to specific physical activities.
  • Awareness among athletes and clinicians of this association is important for diagnosis and management.
