Immunosuppressive therapy for IgA nephropathy (IgAN) shows mixed results, with supportive care often being the first-line treatment. Optimize supportive care before considering immunosuppression for patients at risk of kidney failure.
Area of Science:
- Nephrology
- Immunology
- Clinical Trials
Background:
- The initial pathogenesis of IgA nephropathy (IgAN) offers limited justification for immunosuppressive therapy.
- Immunosuppression may target secondary inflammation or proteinuria in IgAN.
- Current evidence for immunosuppressive therapy in IgAN is inconsistent.
Purpose of the Study:
- To evaluate the efficacy of immunosuppressive therapy versus supportive care in IgA nephropathy.
- To address the limitations of existing clinical trials in IgAN treatment strategies.
- To provide guidance on the optimal sequencing of supportive and immunosuppressive therapies for IgAN patients.
Main Methods:
- Review of randomized clinical trials on corticosteroid monotherapy, mycophenolate mofetil monotherapy, and combination immunosuppressive therapy in IgAN.
- Analysis of studies evaluating supportive therapy for IgAN.
- Comparison of outcomes, including proteinuria and renal failure, in IgAN patients.
Main Results:
- Some trials suggest benefits of immunosuppression for proteinuria or renal failure in IgAN, but results are not universal.
- Many IgAN studies were designed with outdated supportive care recommendations.
- Recent studies show benefits of supportive therapy, but head-to-head comparisons with immunosuppression are lacking.
Conclusions:
- Optimize supportive therapy for IgA nephropathy patients first.
- Reserve immunosuppressive medication for IgAN patients who do not respond to supportive care and remain at risk for kidney function decline.
- Ongoing clinical trials are needed to resolve the therapeutic dilemma in IgAN management.
