Summary

Breast tuberculosis, a rare extrapulmonary form, requires consideration in differential diagnoses for breast diseases, especially in high-incidence areas. Prompt diagnosis and HRZE treatment led to significant lesion improvement in two sisters.

Background:

  • Breast tuberculosis (BTB) is an uncommon manifestation of extrapulmonary tuberculosis.
  • A family history of pulmonary tuberculosis was noted in other siblings of the presented cases.
  • High incidence of tuberculosis in the region necessitates considering BTB in differential diagnoses.

Observation:

  • Two sisters (26/F, 27/F) presented with breast mass, enlargement, pain, and discharge.
  • Initial ultrasonography suggested fibrotic mastopathy and inflammation, masking the tuberculous etiology.
  • Diagnosis was confirmed via excisional biopsy, leading to anti-tuberculosis treatment (HRZE regimen).

Findings:

  • The first case, with concurrent pulmonary tuberculosis, showed complete resolution of breast lesions after HRZE treatment.
  • The second case, with isolated mammary tuberculosis, exhibited marked improvement of breast lesions.
  • Both patients are currently under follow-up and remain asymptomatic.

Implications:

  • Breast tuberculosis should be included in the differential diagnosis for breast lesions, particularly in tuberculosis-endemic regions.
  • Early diagnosis and appropriate anti-tuberculosis therapy are crucial for successful management and resolution of BTB.
  • This study highlights the importance of considering rare presentations of tuberculosis in clinical practice.
