Summary

Severe anti-Duffy allo-immunization in a newborn required intrauterine exchange transfusions. This rare condition, Duffy system allo-immunization, can cause serious hemolytic disease of the newborn, but treatments have improved outcomes.

  • The Duffy blood group system is clinically significant, ranking fourth after ABO, Rh, and Kell systems.
  • Anti-Duffy antibodies (anti-Fya) are IgG immune antibodies that can cause severe transfusion reactions and hemolytic disease of the newborn (HDN).
  • Duffy allo-immunization is rare, with only 25 reported cases of HDN, but carries significant risk.
