Alendronate treatment reduced tartrate-resistant acid phosphatase 5B (TRACP 5b) activity in osteoarthritic bone, particularly in well-supplied intertrochanteric areas. This indicates bisphosphonates are more effective in these regions.
Area of Science:
- Biochemistry
- Orthopedics
- Pharmacology
Background:
- Osteoarthritis (OA) is a degenerative joint disease characterized by bone remodeling.
- Tartrate-resistant acid phosphatase 5B (TRACP 5b) is a key marker of osteoclast activity, crucial in bone resorption.
- Bisphosphonates, like Alendronate, are commonly used to treat bone disorders by inhibiting osteoclast function.
Purpose of the Study:
- To quantify the activity of TRACP 5b in osteoarthritic bone specimens from patients treated with Alendronate.
- To investigate the effect of Alendronate on TRACP 5b levels in different regions of the femoral head.
- To determine the efficacy of bisphosphonates in specific bone areas affected by osteoarthritis.
Main Methods:
- A randomized controlled study involving 12 patients undergoing total hip replacement.
- Patients were divided into a bisphosphonate (Alendronate) group and a control group.
- Femoral head samples (anterior A1 and intertrochanteric A2) were analyzed using immunoassay for TRACP 5b activity and histological staining for TRACP-positive cells.
Main Results:
- Alendronate treatment significantly decreased TRACP 5b enzyme activity in both anterior femoral head (A1) and intertrochanteric (A2) areas compared to controls.
- TRACP 5b activity was significantly higher in A1 compared to A2 in the bisphosphonate group, but not in the control group.
- Histological examination confirmed fewer TRACP-positive cells in bisphosphonate-treated bone sections, corroborating the enzyme activity findings.
Conclusions:
- Bisphosphonates, specifically Alendronate, effectively reduce TRACP 5b activity in osteoarthritic bone.
- The reduction in TRACP 5b activity by bisphosphonates is more pronounced in the intertrochanteric area (A2) than in the anterior femoral head (A1).
- These findings suggest that bisphosphonates may be more effective in well-vascularized bone regions, highlighting potential differences in therapeutic response within osteoarthritic bone tissue.

Hormones and Bone Tissue
Hormones That Influence Osteoblasts and/or Maintain the Matrix
Several hormones are necessary for controlling bone growth and maintaining the bone matrix. The pituitary gland secretes growth hormone (GH), which, as its name implies, controls bone growth. This happens in several ways: first, it triggers chondrocyte...
Bone Disorders
Bone deposition is also affected by the levels of sex hormones like estrogen and testosterone that promote osteoblast activity and bone matrix synthesis. When the level of these hormones decreases due to aging, it causes a reduction in bone deposition. As a result, bone resorption by osteoclasts...
