Summary

EDTA-dependent antibodies can cause pseudothrombocytopenia, a false low platelet count. Switching to citrate blood tests can reveal normal platelet levels, avoiding misdiagnosis and unnecessary treatments.

Area of Science:

  • Hematology
  • Clinical Pathology
  • Immunology

Background:

  • Thrombocytopenia, a condition characterized by a low platelet count, can lead to serious health complications.
  • Accurate platelet counting is crucial for diagnosis and treatment planning.

Observation:

  • Four patients presented with a falsely low platelet count when using EDTA (ethylenediaminetetraacetic acid) as an anticoagulant.
  • Platelet counts normalized when blood samples were analyzed using citrate as an anticoagulant.

Findings:

  • The observed pseudothrombocytopenia was attributed to the presence of "EDTA-dependent" platelet antibodies.
  • These antibodies caused platelet clumping or destruction in the presence of EDTA, leading to inaccurate counts.

Implications:

  • Distinguishing pseudothrombocytopenia from true thrombocytopenia is critical to prevent unnecessary medical interventions.
  • Misdiagnosis can lead to inappropriate treatments, delayed surgical procedures, and patient anxiety.
  • Implementing alternative anticoagulant testing (e.g., citrate) is essential for accurate platelet evaluation in suspected cases.
