Background:

Functional endoscopic sinus surgery (FESS) is a common procedure for sinonasal diseases.
Oculomotor nerve (cranial nerve III) palsy is a rare but serious complication following head and neck surgery.
Understanding potential neurological complications of FESS is crucial for patient safety.

Observation:

A patient developed partial oculomotor nerve palsy post-FESS.
The palsy presented without evidence of anatomical damage to extra-ocular muscles.
Clinical presentation and progression of the palsy were closely monitored.

Findings:

The oculomotor nerve palsy was confirmed to be an unusual sequela of FESS.
Pathogenic mechanisms, potentially involving inflammatory or ischemic processes, were explored.
