Summary

Corticosteroids reduce the risk of death and severe disability in tuberculous meningitis patients. Routine use is recommended for HIV-negative individuals, but evidence for HIV-positive patients is insufficient.

Background:

  • Tuberculous meningitis (TBM) is a severe form of tuberculosis with high mortality and disability rates.
  • The efficacy of corticosteroids as an adjunct treatment for TBM remains controversial.

Purpose of the Study:

  • To evaluate the impact of corticosteroids on mortality and severe disability in patients with tuberculous meningitis.

Main Methods:

  • Systematic review and meta-analysis of randomized controlled trials.
  • Searched multiple databases including Cochrane, MEDLINE, and EMBASE.
  • Analyzed data using relative risks and a fixed-effect model.

Main Results:

  • Seven trials with 1140 participants were included.
  • Corticosteroids significantly reduced the risk of death (RR 0.78, 95% CI 0.67 to 0.91).
  • Corticosteroids also reduced the risk of death or disabling neurological deficit (RR 0.82, 95% CI 0.70 to 0.97).

Conclusions:

  • Corticosteroids should be routinely used in HIV-negative TBM patients.
  • Evidence is insufficient to recommend corticosteroids for HIV-positive TBM patients.
