Background:

  • Ankylosing spondylitis (AS) is a chronic inflammatory rheumatic disease.
  • Physiotherapy is a key component in managing AS.

Purpose of the Study:

  • To systematically review the scientific evidence on the effectiveness of physiotherapy interventions for ankylosing spondylitis management.

Main Methods:

  • Searched multiple databases (CENTRAL, MEDLINE, EMBASE, AMED, CINAHL, PEDro) up to January 2007.
  • Included randomized and quasi-randomized controlled trials involving AS patients and physiotherapy interventions.
  • Assessed outcomes including pain, stiffness, spinal mobility, physical function, and patient global assessment.

Main Results:

  • Individualized or supervised exercise programs showed benefits over no intervention (low-quality evidence).
  • Supervised group physiotherapy demonstrated superior outcomes in spinal mobility and patient global assessment compared to home exercise programs (moderate-quality evidence).
  • Combined inpatient spa-exercise therapy with outpatient physiotherapy yielded significant improvements in pain, physical function, and patient global assessment compared to physiotherapy alone (moderate-quality evidence).

Conclusions:

  • Individual home-based or supervised exercise programs are more effective than no intervention for AS.
  • Supervised group physiotherapy offers greater benefits than individual home exercises.
  • Combined inpatient spa-exercise therapy followed by group physiotherapy is more effective than group physiotherapy alone for AS management.
