Background:

  • Narcolepsy is a central nervous system disorder characterized by excessive daytime sleepiness (EDS) and cataplexy.
  • Narcolepsy significantly impairs patients' quality of life.
  • Antidepressants are commonly prescribed for cataplexy and sometimes for EDS in narcolepsy patients.

Purpose of the Study:

  • To evaluate the efficacy of antidepressant drugs in treating EDS and cataplexy in narcolepsy.
  • To assess the impact of antidepressants on quality of life and their associated side effects.

Main Methods:

  • Searched multiple databases (Cochrane, MEDLINE, EMBASE, PsycINFO, CINAHL) up to 2007.
  • Included randomized or quasi-randomized controlled trials comparing antidepressants to placebo, no treatment, or other antidepressants.
  • Two reviewers independently assessed trial quality and extracted data; meta-analysis was not performed due to study heterogeneity.

Main Results:

  • Five trials (246 participants) were included; methodological quality was generally unclear.
  • Femoxetine showed no significant effect on EDS but reduced cataplexy; viloxazine reduced both EDS and cataplexy.
  • Ritanserin did not demonstrate effectiveness in reducing EDS or cataplexy compared to placebo.

Conclusions:

  • There is a lack of high-quality evidence supporting the effectiveness of antidepressants for narcolepsy or improving quality of life.
  • Despite clinical recommendations, evidence for antidepressants improving cataplexy is scarce.
  • Well-designed randomized controlled trials are necessary to establish the role of antidepressants in narcolepsy management.
