  • Esthetic success assessed using the papillary index after 3 months of prosthetic loading.
  • Periodontal sounding and radiographs used to measure interproximal bone height and papilla presence.

    • Main Results:

    • The immediate flapless implant group (ILA) achieved the highest average papillary index score (2.6).
    • Papilla presence was 100% when the distance between crown contact point and interproximal bone was <5mm.
    • Papilla presence decreased significantly to 46.5% at 6mm and 24% at >7mm bone-to-contact distance.

    Conclusions:

    • Immediate flapless implantation offers superior esthetic results in the maxillary anterior region.
    • Maintaining interproximal bone height below 5mm is critical for optimal papilla formation and esthetics.
    • Esthetic outcomes of dental implants are significantly influenced by papilla height and bone levels.
