Summary

Prostatic stromal sarcoma (PSS) is a rare and aggressive tumor. This case highlights the challenges in diagnosing PSS, a malignant mesenchymal tumor of the prostate, and its poor prognosis despite treatment.

Background:

Prostatic stromal sarcoma (PSS) is an exceptionally rare malignant mesenchymal tumor arising in the prostate gland.
This condition is infrequently documented, making its clinical presentation and behavior challenging to ascertain.

Observation:

A 33-year-old male presented with perineal pain, elevated prostate-specific antigen (PSA), and a large retrovesical mass with lung metastases on CT scans.
Initial transrectal needle biopsy suggested a mesenchymal tumor, necessitating a larger specimen for definitive diagnosis.
Palliative transurethral resection (TUR-P) was performed for urogenital obstruction, yielding the final diagnosis of PSS.

Findings:

Histological examination revealed a malignant stromal element with high cellularity, mitotic activity, and pleomorphism, consistent with PSS.

The tumor contained yellowish gelatinous material.

The patient received VIP (etoposide, ifosfamide, cisplatin) chemotherapy, but the tumor progressed, leading to rapid deterioration and death.

Implications:

This case underscores the aggressive nature of prostatic stromal sarcoma.
Accurate histological and immunohistochemical analysis is crucial for diagnosing PSS.
The rarity and aggressive behavior of PSS present significant challenges in patient management and treatment strategies.
