[New approaches to surgical treatment of Crohn's disease]

The article presents the results of a study on the effectiveness of optimal medical treatment of Crohn's disease of the small and large intestines. The study was based on the results of treatment of 222 patients with Crohn's disease of the intestine. The patients were divided in two groups: a control group and an experimental one. The principal difference between these two groups consisted in the choice of different medications used for basic therapy of the disease and the choice of different approach to the treatment. In the case of the experimental group, the choice of an optimal form of medical treatment and the application of highly effective medicines (mesalazine and budezonide) in basic therapy led to a significant improvement in the results of treatment: reduction by one-third of the indicators for surgical treatment, reduction by 32.8% of limited resection of the intestine, decrease in 26.8% of the frequency of post-operative complications, decrease in 7.2% of relapses and reduction by 14.9% in mortality. The optimal means of surgical treatment of Crohn's disease of the intestine is onestage resection and colectomy is advised only in cases of total colitis.
