Exercise in severe COPD: is walking different from stair-climbing?

- Stair-climbing induced significantly more dyspnea (BDS 6.5 vs. 5.5), higher blood lactate (1.1 vs. 0.3 mmol/l), and greater pH drop (-0.05 vs. -0.03) compared to walking.
- Stair-climbing led to prolonged lung hyperinflation (4.4% TLC increase), unlike the 6MWT.
- No correlation was found between 6MWT distance and stair-climbing time.
Conclusions:
- Stair-climbing elicits more severe physiological stress, including dyspnea and lung hyperinflation, than the 6-minute walking test in severe COPD.
- 6MWT performance does not predict stair-climbing capacity in these patients.
- The 6MWT may not fully capture the functional limitations related to activities like stair-climbing in severe COPD.
