Summary

Low grade diffuse gliomas have distinct cellular compositions. Morphometric and immunohistochemical analyses help differentiate diffuse astrocytomas, oligodendrogliomas, and oligoastrocytomas based on cellularity and protein expression.

Area of Science:

  • Neuro-oncology
  • Pathology
  • Biomedical Image Analysis

Background:

  • Low-grade diffuse gliomas are challenging brain tumors due to their infiltrative nature.
  • Accurate classification and understanding of cellular heterogeneity are crucial for effective treatment strategies.

Purpose of the Study:

  • To clarify the cellular composition and histopathological features of low-grade gliomas.
  • To differentiate between diffuse astrocytomas (DA), oligodendrogliomas (OL), and oligoastrocytomas (OA) using quantitative methods.

Main Methods:

  • Examined 78 low-grade glioma cases (21 DA, 36 OL, 21 OA) based on WHO classification.
  • Subdivided oligodendrogliomas into three types (I, II, III) based on morphological characteristics.
  • Utilized image analysis software ('Gunmetry') for morphometric analysis (cellularity, nuclear form factor, conditional entropy) and immunohistochemical analysis for antigen expression (GFAP, nestin, p53, Olig2).

Main Results:

  • Morphometric data showed significant differences in cellularity (OL type II vs. DA) and nuclear pleomorphism (OL type III vs. DA).
  • Immunohistochemistry revealed significant differences in GFAP, nestin, and p53 expression between DA and OL.
  • Olig2 expression was mutually exclusive with GFAP and nestin in most glioma cells; coexpression of nestin and GFAP occurred in DA and OA, but not in OL.

Conclusions:

  • Each glioma type exhibits a characteristic proportion of cells expressing GFAP, nestin, and Olig2.
  • Diffuse gliomas share cellular compositions in varying ratios, distinguishable by morphometric analysis.
  • Morphometric and immunohistochemical analyses provide valuable insights for differentiating low-grade glioma subtypes.
