Summary

A new laparoscopic sewing device, Endosew, significantly reduces suturing time by 2.4-fold compared to manual methods. While nearly identical in quality, machine sutures required occasional adjustments for leaks, but time savings outweigh this.

Area of Science:

  • Urology
  • Surgical Innovation
  • Minimally Invasive Surgery

Background:

  • Laparoscopic cystectomy and neobladder procedures are lengthy, with suturing being a major time consumer.
  • Improving suturing efficiency is crucial for advancing laparoscopic techniques.

Purpose of the Study:

  • To develop and evaluate a novel sewing device for laparoscopic running sutures.
  • To assess the time efficiency and quality of sutures placed by the new device compared to manual suturing.

Main Methods:

  • A comparative study using six pigs, evaluating hand-sewn versus machine-sewn running sutures in the ileum.
  • Suturing time, closure tightness, and macroscopic/microscopic quality were assessed post-explantation at 3, 6, and 9 days.

  • No animal mortality or postoperative leaks occurred.
  • Macroscopic and microscopic examination revealed no significant difference in suture quality between hand and machine methods.
  • Machine suturing reduced the time to place 1 cm of running suture from 7.2 minutes (hand) to 3 minutes (machine).
  • Four instances of leaks required adjustment for machine sutures, while hand sutures were initially tight.

Conclusions:

  • The Endosew device offers a 2.4-fold reduction in laparoscopic running suture time.
  • Suture quality is comparable between manual and machine methods.
  • The time saved using the Endosew device justifies its use, despite occasional minor adjustments for leaks.
