  • Understanding the long-term health implications, including mortality, of OAG is crucial, particularly in diverse populations.
  • Previous research has explored OAG and mortality, but specific data in Black populations are limited.

Purpose of the Study:

  • To investigate the association between open-angle glaucoma (OAG) and all-cause and cause-specific mortality.
  • To examine the relationship between ocular hypertension and mortality.
  • To assess these associations in a cohort of Black adults over a 9-year follow-up period.

Main Methods:

  • A population-based cohort study of 4092 Black participants aged 40-84 years was conducted.
  • Open-angle glaucoma (OAG) and ocular hypertension were diagnosed using standardized ophthalmic examinations and grading.
  • Mortality data were collected from death certificates, and Cox proportional hazards regression was used for analysis.

Main Results:

  • After 9 years, 19% of participants died. Overall OAG at baseline was not associated with mortality.
  • Cardiovascular mortality showed a trend toward increase in individuals with previously diagnosed/treated OAG (RR, 1.38; P = .07).
  • A significant increase in cardiovascular mortality was observed with timolol maleate treatment for OAG (RR, 1.91; P = .04), and a similar trend was noted for ocular hypertension (RR, 1.28; P = .06).

Conclusions:

  • In this Black population, diagnosed/treated OAG and ocular hypertension were associated with increased cardiovascular mortality.
  • The elevated cardiovascular mortality linked to timolol maleate treatment warrants further investigation.
  • These findings highlight potential systemic health risks associated with glaucoma and its treatment.
