Robot-assisted laparoscopic cystectomy using the da Vinci Surgical System is safe and effective for bladder cancer. This study shows promising short-term functional and oncological outcomes with acceptable operating times.

Area of Science:

  • Urology
  • Surgical Oncology
  • Robotic Surgery

Context:

  • Robot-assisted surgery is gaining traction, but concerns about operating times and long-term oncological outcomes for laparoscopic cystectomy persist.
  • Previous experience with robot-assisted laparoscopic prostatectomy provided a foundation for this study.

Purpose:

  • To evaluate the technical feasibility, functional outcomes, and short-term oncological results of robot-assisted laparoscopic radical cystectomy (RALRC) using the da Vinci Surgical System (DVSS).

Summary:

  • 27 patients underwent RALRC between 2004-2006 for muscle-invasive bladder cancer or radiation-induced bladder shrinkage.
  • Mean operating time was 340 minutes with a mean blood loss of 301 mL. Negative surgical margins were achieved in all but one case.
  • Short-term follow-up (mean 10.2 months) revealed low complication rates, with good functional outcomes for erectile function and urinary continence.

Impact:

  • Robot-assisted laparoscopic cystectomy demonstrates safety and efficacy, achieving satisfactory functional and oncological results within acceptable timeframes.
  • This technique offers a viable option for patients requiring radical cystectomy, addressing previous limitations of longer operating times.
