Bisphosphonates effectively reduced pain and improved bone lesions in chronic recurrent multifocal osteomyelitis (CRMO) patients. This offers a safe, adjunctive treatment option when standard therapies fail.
Area of Science:
- Pediatric rheumatology
- Bone metabolism
- Pharmacology
Background:
- Chronic recurrent multifocal osteomyelitis (CRMO) is a rare, painful bone inflammatory condition.
- Conventional treatments like NSAIDs are often insufficient for managing CRMO symptoms and progression.
Observation:
- A study evaluated pamidronate, a bisphosphonate, in five CRMO patients with persistent pain and functional loss.
- Patients received pamidronate infusions over 12 to 42 months.
Findings:
- Four out of five patients experienced significant pain reduction after the first pamidronate infusion.
- Radiological evidence showed marked improvement in bone lesions for these four patients.
- Symptomatic improvement was sustained throughout the treatment period.
Implications:
- Bisphosphonates demonstrate safety and efficacy as an adjunctive therapy for CRMO.
- This treatment is particularly beneficial for patients unresponsive to anti-inflammatory agents or those with active lesion expansion.
- Pamidronate offers a promising therapeutic avenue for refractory CRMO cases.
