Summary

Septic discitis, a serious spinal infection, should be suspected in elderly patients with Paget's disease who experience severe back pain and elevated inflammatory markers after a fall.

Background:

Paget's disease of bone is a chronic condition that can predispose individuals to complications.
Elderly patients often present with complex medical histories and atypical symptoms.

Observation:

An elderly female patient with Paget's disease presented with acute, severe back pain following a fall.
Initial presentation included elevated inflammatory markers and a suboptimal response to standard pain management.

Findings:

Magnetic Resonance Imaging (MRI) confirmed the diagnosis of septic discitis, a spinal infection.
The clinical presentation, particularly the elevated inflammatory markers, was indicative of an infectious process.
