Summary

Diffusion tensor imaging (DTI) reveals subtle abnormalities in normal appearing brain tissue (NABT) in early relapsing-remitting multiple sclerosis (RR MS). However, DTI did not show significant longitudinal changes over two years in these patients.

Background:

  • Relapsing-remitting multiple sclerosis (RR MS) is characterized by neurological deficits.
  • Abnormalities in normal appearing brain tissue (NABT) are observed in RR MS.
  • The longitudinal evolution of NABT abnormalities in early RR MS is not well understood.

Purpose of the Study:

  • To investigate changes in Diffusion Tensor Imaging (DTI) parameters over two years in early RR MS patients.
  • To evaluate the potential of DTI as a quantitative longitudinal marker for early RR MS.

Main Methods:

  • A longitudinal study involving 28 RR MS patients and 15 controls.
  • Annual DTI scans were performed over two years.
  • Analysis of fractional anisotropy (FA) and mean diffusivity (MD) histograms in NABT and whole brain tissue.

Main Results:

  • Baseline differences in FA between patients and controls were observed.
  • At two years, differences in MD were noted between patients and controls.
  • No significant longitudinal differences in the rates of change of DTI parameters were found between patient and control groups.

Conclusions:

  • Subtle DTI abnormalities exist in NABT in early RR MS.
  • Global DTI assessment of NABT lacks significant longitudinal change in early RR MS.
  • DTI may have a limited role in monitoring the early disease course of RR MS longitudinally.
