Summary

This case report details an unexpected intraretinal extension of an epiretinal membrane during surgery. Microscopic analysis revealed retinal pigment epithelium-like cells and fibroblasts within the membrane.

  • Epiretinal membranes (ERMs) can cause visual distortion.
  • Secondary ERMs can form after procedures like laser coagulation for diabetic retinopathy.

  • During vitrectomy for ERM removal, an unexpected subretinal extension was noted.

Findings:

  • Histopathology revealed the membrane had both epiretinal and intraretinal components.
  • Electron microscopy identified connective tissue, capillaries, and retinal pigment epithelium-like and fibroblast-like cells.

Implications:

  • This case highlights a rare complication of ERM surgery.
